THE WHITE-GLOVE STANDARD, DELIVERED AT HOME

Three things every family deserves. We won't do it any other way.

Anticipatory Care, Not Reactive

A Registered Nurse personally writes every care plan and stays close throughout. Our care teams are trained to notice before you have to ask. Attention to detail isn't a feature. It's the whole standard.

A Dedicated Team Who Remember

No rotating strangers at the door. Your loved one is thoughtfully matched with a consistent care team who learn their routines, their preferences, and the small things that make a house feel like home.

Ready Within 24 Hours

When a family needs us, we do not make them wait. From the first call to the first visit, we move with the quiet efficiency and discretion of the best hospitality on earth.

Concierge care, tailored three ways.

One standard of care. Three ways families can access it.

Tailored for the families of Bloomfield Hills, Birmingham, and the Grosse Pointes who expect the highest standard of care at home.

VA Home Care for Veterans

Homemaker · Home Health Aide · Respite Care

It is an honor to care for the men and women who served this country, and to do so with the standard their service has earned. Care Plan Inc is a VA-registered provider, coordinating directly with VA Medical Centers on behalf of the veterans and families we serve. Homemaker care, Home Health Aide services, and Respite Care for the families who carry the weight day and night. We handle the coordination. You focus on your loved one.

Care Plan Inc is an independent provider and does not determine VA eligibility. We guide veterans and families so they can make informed choices.

Michigan Home Help Program

Get Paid to Care for a Family Member

Accessible care, held to the same Care Plan Inc standard. The Michigan Home Help Program is a Medicaid-funded benefit that pays approved family members, friends, and neighbors to care for a loved one at home. We guide families through the full MDHHS and CHAMPS enrollment process, from eligibility to first paycheck. Most participants qualify for up to 180 hours a month.

Four families. One standard.

Why families choose home, always.

Families in Bloomfield Hills, Birmingham, and the Grosse Pointes consistently choose in-home care over every other option. Here's why.

Family choosing home care

At home, with everything familiar.

Your loved one stays in the place they know, with care shaped around their routines.

Independence on their own terms.

Life continues at their pace, not a facility's schedule.

A dedicated care team.

One consistent team, one tailored care plan, nothing rushed, nothing shared.

The gift of time, for everyone.

Families stay connected through proactive communication. Adult children get to be sons and daughters again.

Better outcomes, at home.

Healing, dignity, and wellbeing are consistently stronger when care happens in familiar surroundings.

In the words of the families we serve.

About Care Plan Inc

+

What makes Care Plan Inc. different from other home care agencies?

+
Three things, and we won't do it any other way. First, every family receives dedicated oversight from our on-staff nursing team. Your care is professionally guided from day one. Second, we specialize in transitional care and recovery, so when a loved one comes home from the hospital, every detail of their return is anticipated and managed. Third, we serve Southeast Michigan's most distinguished communities exclusively, bringing the same discretion and attention to detail you'd expect from the finest hospitality in the world.

What does concierge care mean?

+
It means we shape everything around your family's life, not our schedule. Flexible timing. A tailored care plan written by our nursing team. A consistent care team who learns your loved one's routines, preferences, and the small things that make a house feel like home. Proactive communication so you're never left wondering. And the kind of anticipatory attention that turns home care into something your family actually looks forward to. Every preference remembered. Every detail considered.

Are your caregivers bonded and insured?

+
Yes. Every member of our care team is fully bonded and insured, with comprehensive liability coverage. Your family is completely protected, and your home is treated with the same respect and care you'd expect from the finest service providers.

What qualifications do your caregivers have?

+
Every caregiver on our team is a credentialed care professional with specialized training in transitional and recovery care. They complete thorough background screenings, skills assessments, and ongoing education. But credentials are only part of the picture. We select our team for warmth, attentiveness, and the ability to make your loved one feel genuinely cared for, not just looked after. Every caregiver works under the direct guidance of our nursing team.

Services & Care

+

What services do you provide?

+
Our signature services cover everything a family needs to feel at ease. Specialized recovery support during the critical first weeks home from the hospital. Personal care including bathing, dressing, grooming, and mobility. Thoughtful companionship and meaningful social connection. Medication reminders to keep schedules safe. Light housekeeping so your loved one's home stays comfortable and welcoming. Meal preparation tailored to dietary needs and personal preferences. Safe transportation to appointments, errands, and the places that matter. And respite care, giving family caregivers the gift of time to rest and recharge. Everything we do is guided by our nursing team.

Do you provide medical or nursing care?

+
Our care is personal, not medical. Our team doesn't administer medications or perform medical procedures. What makes us different is the layer of professional oversight behind every day of care. Our on-staff nursing team writes every care plan, supervises every caregiver, and coordinates with your loved one's doctors to ensure nothing falls through the cracks. It's concierge-level attention with a clinical backbone.

Can you help with dementia or Alzheimer's care?

+
Yes. Many of our caregivers have specialized training in memory care and understand what families navigating cognitive changes truly need: consistency, dignity, and patience. We create personalized approaches that honor your loved one's independence while keeping them safe and comfortable. Our nursing team helps develop the right strategies for each situation, so your family has confidence and your loved one has continuity.

Do you provide transportation services?

+
Yes. Our caregivers can accompany your loved one to appointments, shopping, social outings, and errands. Families in our communities value this deeply. It's one of the simplest ways to protect independence, maintain social connections, and give your loved one the freedom to live on their own terms.

Is there a minimum number of hours required?

+
We shape our services around your needs, whether that's a few hours of recovery support or comprehensive daily care. Our minimum is typically four hours per visit, which gives our team the time to provide meaningful, unhurried attention. But we're flexible, and we'll talk through exactly what makes sense for your family during your consultation.

Do you provide care during nights and weekends?

+
Absolutely. Our concierge model means 24/7 availability. Whether you need overnight recovery support, evening companionship, or weekend care, we're here when your family needs us. We design our schedule around yours, not the other way around. That's the standard.

What happens in a medical emergency?

+
Every member of our care team is trained to respond calmly and effectively. In an emergency, they'll contact 911 immediately, then notify your family and our nursing team. We keep detailed records so first responders have everything they need the moment they arrive. Our nursing staff stays involved throughout, providing guidance until the situation is fully resolved. You'll never be in the dark.

What safety measures do you have in place?

+
Safety isn't a feature we add. It's built into everything. Thorough background screenings for every caregiver, ongoing supervision by our nursing team, personalized home safety assessments, regular training, and 24/7 on-call support. Our anticipatory approach means we identify potential concerns before they become problems. That's the peace of mind our families count on.

Costs & Coverage

+

What does concierge home care cost in Birmingham, Bloomfield Hills, and Grosse Pointe?

+
Our concierge services start at $45/hour, reflecting the caliber of our care team and the nursing oversight that guides every day. This isn't just a caregiver at your door. It's a dedicated team, a tailored care plan, ongoing family communication, and the peace of mind that comes with knowing every detail is being handled. Every family's needs are different, and we'll walk through a clear, transparent quote during your complimentary consultation.

Do you accept insurance or Medicare?

+
Our concierge services are private pay, which gives families maximum flexibility and allows us to maintain the highest standard without insurance limitations. We're also a VA-registered provider for eligible veterans, and we serve families through the Michigan Home Help Program for those who qualify for Medicaid coverage. If you carry long-term care insurance, we can work with your policy as well. During your consultation, we'll help you explore every option available to your family.

How does Michigan Medicaid cover home care services?

+
Through the Michigan Home Help Program, eligible families can access Medicaid-funded in-home care services. The program can even pay approved family members to care for a loved one at home. Our team handles the paperwork and works directly with the state to make enrollment as smooth as possible. We'll verify your eligibility and walk you through every step, so what feels overwhelming becomes manageable.

Service Area

+

Which communities do you serve?

+
We have the privilege of serving Southeast Michigan's most distinguished communities: Bloomfield Hills, Bloomfield Township, Birmingham, all five Grosse Pointe communities (Park, Farms, Shores, Woods, and City), Troy, Franklin, West Bloomfield, Orchard Lake Village, Northville, Plymouth, and Novi. Every client lives in a community we know by name, in a home we treat as our own.

Do you serve areas outside Southeast Michigan?

+
No, and that's by design. We focus exclusively on Southeast Michigan so we can maintain the concierge standard our families expect. We know every community by name. We will never franchise, and we will never dilute what makes this work. That's the only way we know to hold the standard we promise.

Getting Started

+

How quickly can care begin?

+
When a family needs us, we don't make them wait. For urgent situations after a hospital discharge, we can often begin care within 24 hours. Our team moves with quiet efficiency: a nursing assessment, a tailored care plan, and a thoughtfully matched caregiver. For planned care, we recommend starting a few days in advance to ensure the perfect fit.

What happens during the initial consultation?

+
It's a quiet, relaxed conversation. We learn about your family's unique needs, preferences, and concerns. We'll talk through your care goals, walk you through our services, review what options are available, and answer every question on your mind. There's no pressure, no obligation. Just genuine guidance to help your family make the right decision at the right time.

What if we're not satisfied with our caregiver?

+
Tell us, and we'll make it right. If the match isn't what your family needs, we'll move quickly to find a caregiver who fits. Our goal is a relationship built on trust, comfort, and genuine connection. Consistency is our standard, not the exception, and we won't stop until your family has exactly what they deserve.

What if my parent is resistant to accepting help?

+
This is one of the most common concerns families share with us, and we understand completely. Independence matters. Our approach is gentle and gradual. We often start with light companionship or help with a few specific things, letting the relationship develop naturally. Our caregivers are skilled at preserving dignity and making your loved one feel respected, not managed. Many families tell us that once their parent experiences the warmth of a Care Plan caregiver, resistance turns into genuine trust.

Can family members still be involved in care?

+
Always. We see ourselves as partners in your loved one's wellbeing, not replacements. Our caregivers work alongside family members, providing professional support while making sure those precious family connections stay strong. For families, it's the gift of time: the chance to be sons and daughters again, instead of full-time caregivers. You'll always have direct access to our nursing team.

I live out of state. Can you keep me updated on my parent's care?

+
Absolutely. Many of our families have adult children across the country, and we know how important it is to feel connected when you can't be there in person. We provide regular updates however you prefer: phone, email, or text. Our nursing team ensures you always know how your parent is doing and what's happening with their care. Distance doesn't change the standard. You'll have peace of mind no matter where you are.
